He had to keep upping the stakes! Graduating from normal adults to blind old lady to child to John Watson was all part of keeping the pressure on. Repetition would have dulled the response, and more importantly it would have bored Sherlock. There was nothing wanton or gleeful going on there. Pure practicality all the way.

Okay, maybe a little gleeful. But only because it worked like it was supposed to!
